Your General Practitioner can assist you with a Chronic Disease Management Plan that provides a Medicare rebate for 5 Speech Pathology services per calendar year.

1. You have received the service from a registered speech pathologist.
2. The service is a Medicare-eligible service.
3. Your Private Health Insurance plan covers Speech Pathology
